Terms and Conditions for UFA Soccer Academy of Iowa
These terms and conditions apply to all players registered with the UFA Soccer Academy of Iowa (hereafter referred to as “the Club”), whose details are submitted using an online registration form and/or who take part in any footballing activity for the Club. By submitting the registration form or allowing your child to take part in activities organized by the Club, you agree that you and your child will comply with these terms and conditions:
- Payment of Fees:
- You agree to pay fees due to the Club, as specified in the UFA Program and dependent on the level of play and age group.
- You acknowledge that your child attending activities run by the Club after fees have been notified to you shall be accepting of your liability to pay such fees.
- Should you decide that your child will not participate in the club mid-season, you are still liable for full payment of dues.
- Compliance with Codes of Conduct and Policies:
- You agree to comply with any codes of conduct and policies issued by the Club.
- Registration with Affiliates:
- You consent to the Club registering your child with the UFA Soccer Academy and its affiliates.
- Affiliates include leagues, competitions, and third-party providers.
- Photography and Videography Consent:
- You consent to the Club taking photographs and/or videography of your child.
- You agree for such recordings to be used by the Club for promotional purposes, including on the website.
- Communication:
- We may send you emails (including marketing) from the Club at the email address supplied during registration.
- Transfer of Personal Data to Third Parties:
- You give permission to the Club to transfer personal data about you and/or your child, including name, email address, and mobile number, to carefully selected third parties providing services to the Club.
- Examples of such third parties include Soccer Master and Spond.
- Such third parties may contact you in connection with the service they are providing to the Club (including marketing such service) but will not use your details for any other reason.
- Emergency Medical Treatment:
- You consent to a Club representative providing basic first aid and lifesaving aid to your child where necessary.
- In circumstances where the Club is unable to contact you or your nominated emergency contact, you agree that a Club representative may take your child to the hospital and give consent for emergency treatment.
Definitions:
- “Club” means United Youth Academy Foundation INC ( UFA) and its agents, including without limitation its coaches and affiliates.
By submitting the registration form or allowing your child to take part in activities organized by the Club, you acknowledge that you have read, understood, and agreed to these Terms and Conditions. Failure to comply with these terms may result in disciplinary action, including the termination of your child’s participation in the Club’s activities.
